CRACKING THE SCLEROTIC BONE RIM COMPARED TO REAMING +*Elmengaard, B; **Bechtold, JE; *Baas, J; Jakobsen, T; *Søballe, K +*Department of Orthopaedic Surgery, University of Aarhus, Denmark [email protected] INTRODUCTION Revision hip replacement implants have poorer clinical outcome than primary implants. The fixation of the implants is often compromised by the formation of an endosteal sclerotic bone rim during the process of aseptic loosening. The cracking procedure is a bone sparing, low energy surgical technique which produces a controlled local perforation of the sclerotic bone rim. In previous studies, we showed that fixation of revision implants significantly improved by the cracking technique for both titanium (Ti) and hydroxyapatite (HA) coated implants with and without the use of bone graft(1). In this study we compared the cracking technique with the common technique of reaming, which completely removes the sclerotic bone rim. We hypothesized that the crack revision procedure would result in superior mechanical fixation of titanium implants compared to those inserted with the reaming procedure.

Fixation of revision implants is improved by new surgical technique to crack the sclerotic endosteal rim.

We used an experimental model producing a tissue response with a sclerotic endosteal neo-cortical rim associated with implant loosening in humans: a 6 mm PMMA cylinder pistoned 500 m concentrically in a 7.5 mm hole, with polyethylene particles. At a second operation at eight weeks, the standard revision procedure removed the fibrous membrane in one knee, and the crack revision procedure was use...

The long-term outcome of the cemented Weber acetabular component in total hip replacement using a second-generation cementing technique.

We report the long-term outcome of a modified second-generation cementing technique for fixation of the acetabular component of total hip replacement. An earlier report has shown the superiority of this technique assessed by improved survival compared with first-generation cementing. The acetabular preparation involved reaming only to the subchondral plate, followed by impaction of the bone in ...

Clinical Results of Percutaneous Fixation of Pelvic and Acetabular Fractures: A Minimally Invasive Internal Fixation Technique

Background: The pelvic ring fractures (PRF) and acetabular fractures (AF) are among the major orthopedic injuriesassociated with high rates of morbidity and mortality. Open surgical stabilization is the standard treatment for themajority of these complications. Percutaneous minimally invasive surgical stabilization of the fractures has become anaccepted treatment method for th...

Background: The purpose of this retrospective study was to analyze the long-term results of revision ORIF, jointcontracture release, and autogenous bone-grafting in the treatment of distal humerus frac-ture nonunions in older agedpatients with poor bone quality or bone loss who would have been candidates for total elbow arthroplasty. Methods: Seven patients (average age at inde...
